Northwest Tower Crane Services Inc. of Sea Tac and Houston-based Champion Elevators Inc. were each fined $3,900 by the state for serious workplace safety violations.

Beam kills bridge worker

A West Layden man, James P. Burke, 42, was killed when he was struck by a steel beam. A crane was moving the 40 foot beam into place for driving into the ground when the cable holding the beam broke.

It is reported that Mr. Burke was working for CCI Construction on a bridge project.

Crane operator, Julian Aguirre , was killed Monday night when a metal beam fell on him at a South Side steel plant, police said.

Worker injured at site of bridge

A member of a Washington County Highway Department crew clearing logs and debris from the base of the Broadfording Bridge sustained a broken leg Tuesday morning when he was struck by a log, rescuers said.

Using just a little common sense will usually do the trick.

06/21/02: Okinawa, Japan —

It was a hot summer day in May 2002, in Japan (Okinawa). I was picking up ribbon bridge sections. I had already lifted five other sections. By the time the next section was ready, I had to move closer. I was set up over the rear picking up 13,500 lbs and swinging it to the center. The rigger hooked the slings and I tighted the slings and was ready to go. As I started to get closer to the center, the signalman saw the front left outrigger lift up. I tried to correct it, but it was too late.

I stayed in the cab with my hands on the windshield as it went down. Luckly I walked away only just shaking and slight panic.

The crane is a Harnischfeger High Speed High Mobility Crane (HSHMC) 25ton. I have over three years experience with the crane and went to a Marine Corps Journeymans Course.

I hope you can use these photos as a tool for other operators to follow. The investigators never found out what happened, niether do I know. I was set up for 19,100#.

Steel reinforcing rods weighing more than half a ton broke loose as they were being lifted by a crane Monday and rained down on construction workers, killing one, officials said.

The dead man was identified as Richard Ellis, 40. A second man suffered a minor injury.

A Sauk Rapids man, Daniel Kirchner, 40, was working in a ladder bucket extended from a truck, reportedly co-owned by Accurate Electric, a Sauk Rapids-based electrical company.
